Please see below for our full terms and conditions. Please be mindful that we are bound by confidentiality laws. All notes from sessions will be stored either in locked files, or in a password protected online system. Once read, please fill out our acceptance form below, and indicate your selected program. Welcome to TeenCoach

As your child/teen is a client at OutsideIn Teen he/she has certain rights that are important to understand. Coaching services can address issues of a highly personal nature and it is important that you understand your rights in regards to your child/teen(s) records and personal information collected.

Purpose of Collecting and Retaining Information
As part of providing a coaching service, the Coach will need to collect and record personal information that is relevant to your child/teen(s) current situation. Collection of personal information is a necessary part of our coaching service, however, is seen only by OutsideIn teen Coach's and Senior Psychologists. The information is retained in order to document what happens during coaching sessions, and enables the Coach to provide an informed service.

Exchange of Client Information
There may be times where, as part of the Coaching program, it may be helpful for your Coach to liaise with our team of Senior Psychologists, when relevant to your child/teen(s) goals. Please note that signing this form acknowledges your consent to allow us to do so.  

Confidentiality
OutsideIn Teen respects the confidentiality of all clients. OutsideIn Teen will keep personal information such as numbers, addresses, and other relevant information to the service secure, in accordance with the Australian Psychological Code of Conduct. With the allowance of certain exceptions described below, your child/teen has the right to the confidentiality of the information that is shared. You are assured that all personal information gathered by the Coach during the provision of the service will remain confidential and secure. However, it is important to know there are exceptions in which Coach's will exercise their duty to waive confidentiality.

This can occur when:
1. The information you and your child/teen has provided to the psychologist is subpoenaed (officially requested) by a court of law
2. Failure to disclose certain information would place your child/teen or another person at serious risk of harm
3. Your prior approval has been obtained to:
     a. Provide a written report to another professional or agency, e.g. a GP or a school;
     b. Discuss the material with another person, e.g. a teacher

Invoices and Payment
Upon your acceptance of our Terms and Conditions we invoice services as follows:
TeenStart programs are invoiced upfront in full at $550. 
TeenTranform programs are invoiced 50% upfront, and the final 50% is invoiced half way through the coaching program, totalling $1750. 
If you request for a session to be held at an alternate location to our office at 60 North Terrace, Kent Town, additional cost may apply. All invoices are payable via EFT within 14 days from invoice date. After written notification, if any account is not paid interest on any outstanding balance may be incurred. 

Cancellation Policy
We know your time is valuable, and ours is too. If, for some reason, you need to cancel or postpone your appointment, please give at least 48 hours notice. If you cancel a regular appointment with 48 hours notice then a cancellation fee will not apply. If you cancel an appointment at short notice, i.e. less than 48 hours prior to your appointment, 30 minutes will be deducted from your Coaching program. A no show or missed session will incur the full session time deducted from the coaching program. If a cancellation was made due to unavoidable circumstances, then we will try our best to reschedule the appointment.
